Application Checklist

The more information you can give us using the online application, the faster it will be for us to process your request. However, we can also collect any missing information during the phone interview.

This is what we will be asking for:

  • Eligibility.Email address where you received your invitation to apply and 6-digit code; date of birth, affirmation of membership in The Mother Church.
  • Applicant Information. Name, address, email, phone number(s).
  • Personal Representative Information. Contact information for the person completing the form if not the applicant. This person should have a Power of Attorney (POA).
  • Financial Information. Before starting the online application, please print and complete the Financial Worksheet that documents annual household income, cost of housing, assets, and liabilities. You will not submit this worksheet, but you will need to refer to it while filling out the application.
  • Documentation. If applicable to your specific situation, we may ask you to submit copies of:
